The Sports Physical Forms and Procedures have changed! See below for details!
All students wishing to be a part of any WINTER Tighe School Sports Team must complete the following forms and hand in to the School Health Office by FRIDAY, OCTOBER 25, 2024 to allow adequate time for our district physician to review and approve student eligibility. Students will not be permitted to attend try-outs until they receive clearance by our School Physician. It is important to get your forms submitted as early as possible to ensure your child's ability to participate.

Complete Sports Physical Packet
Click on the above link for the complete set of paperwork required for your child to participate in school sports at Tighe School. This package includes the following documents:
- **NEW** Preparticipation Physical Evaluation Form (PPE) - To be completed by parent/guardian AND home pediatrician (pages 1-4). These pages stay at healthcare provider's office. Do not return to school.
- **NEW** Preparticipation Physical Evaluation Medical Eligibility Form - To be completed by healthcare provider and is the ONLY form that the healthcare provider signs that should be returned to school nurse. Please note that it needs to be signed in TWO places by your healthcare provider.
- Health History Update Questionnaire (Required when PPE is older than 90 days)
- Sudden Cardiac Death in Young Athletes Fact Sheet
- Sudden Cardiac Death Pamphlet Sign-Off Sheet (Parent/Guardian Signature Required)
- Opioid Use and Misuse Educational Fact Sheet
- Use and Misuse of Opioid Drugs Fact Sheet Parent/Guardian Sign Off (Parent/Guardian Signature Required)
- Sports-Related Concussion and Head Injury Fact Sheet and Parent/Guardian Acknowledgement Form (Parent/GuardianSignature Required)
- Sports-Related Eye Injuries Fact Sheet
- Margate City School District Transportation Consent & Waiver for Sports Activities (Parent/Guardian Signature Required)
Health History Update Questionnaire
This form must be completed when the student's physical exam is older than 90 days, and is generally required at the start of each athletic season.
Paper copies of the above documents are available in the School Health Office.
